Semaglutide dosing is not a single fixed amount — it follows a titration protocol in which the dose is increased gradually over several months. This approach was established through clinical research as the method that achieves effective pharmacological concentrations while minimising the gastrointestinal side effects that occur when higher doses are introduced abruptly. Understanding the rationale for dose titration and the dosing parameters used in research is essential context for anyone studying this compound.

Why Dose Escalation Is Necessary

The area postrema (the brain’s primary nausea-triggering zone) expresses high levels of GLP-1R, and its sudden sustained activation produces nausea in a large proportion of individuals.

Research has established that gradual dose escalation allows neurological adaptation — the receptor system adjusts to the new level of stimulation, and the nausea response diminishes. Jumping directly to a maintenance dose bypasses this adaptation period and dramatically increases the incidence of severe GI side effects and treatment discontinuation.

The standard reconstitution protocol uses bacteriostatic water (BAC water), which contains 0.9% benzyl alcohol as a preservative. This allows the reconstituted solution to be stored for an extended period (typically up to 28 days at refrigerator temperature) rather than requiring immediate use.

For reconstitution, sterile technique should be observed throughout. The solvent should be introduced slowly along the side of the vial rather than directly onto the powder. Gentle swirling (not shaking) is used to dissolve the peptide. Vigorous agitation should be avoided as it can cause the fatty acid chain of semaglutide to aggregate.
